Summary Care Record

The Summary Care Record (SCR). It is an electronic record which contains information about the medicines you take, allergies you suffer from and any other important medical information.It can be seen and used by authorised staff in other areas of the health and care system involved in the patient's direct care. Access to SCR information means that care in other settings is safer, reducing the risk of prescribing errors. It also helps avoid delays to urgent care.

 

At a minimum, the SCR holds important information about;

  • current medication

  • allergies and details of any previous bad reactions to medicines

  • the name, address, date of birth and NHS number of the patient

The patient can also choose to include additional information in the SCR, such as details of long-term conditions, significant medical history, or specific communications needs.


Why do I need a Summary Care Record?

 

Storing information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.mThis information could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you.

 

Who can see it?

Only healthcare staff involved in your care can see your Summary Care Record. 

 

Do I have to have one?

No, it is not compulsory. If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery. You can use the form at the foot of this page.
