Surgery Modifications

Modifications to the Churchfield Surgery main entrance, waiting room and reception area are due to start in February. The practice hopes to minimise any disruption, but apologises for any which may occur. Once the modifications are complete there should be improved disabled access, privacy for patients and a redeveloped waiting area. Whilst work is being carried out, the capacity of the car parking area will be reduced.  There will be a period when patients will be required to access the surgery from the side entrance. If patients are of limited mobility please inform the receptionist when making an appointment as it may be that an appointment at the Sandsend Surgery would be more appropriate.

Travel Vaccinations

Please note: if you are planning to travel abroad and you thnnk you may need vaccinations. Please fill out our travel assessment form and hand it back to the surgery in good time. Some vaccinations may take a while to be affective. We want to make sure you are fully covered for your trip!

Updated email communications

We have recently updated our email communications.


For administration enquires please contact hrwccg.sandsmp-admin@nhs.net. Please note this should not be used for medical queries as this email is not manned 24/7 and will only be answered throughout a working day.


If you have any dispensing related queries please email ssmp.dispensing@nhs.net. This should be used for dispensing queries or medication issues only. Your query will then be directed to the medication team.


For ordering repeat medications and booking appointments etc. Please use patient the patient access system as it is more appropriate and secure.

E-Consultations September 2018

From September 2018 we will be trialling an E-Consultations service. This will allow you to securely message the practice to ask your doctor a question. Your doctor is then able to reply to you and send through any documents you might need. Please note that this service is not for urgent medical enquiries or if you are unwell. After the trial period this service may not continue.

New fees

From 25th May there has been some small changes to our private fees at the surgery. This is due to the new GDPR regulations. To see the changes please go to the 'Private Fees' Tab.  

NHS health checks

We are currently sending out letters to patients that are eligble for free NHS health checks. If you are between 40-74 and have not had a health check in the last 5 years you should be recieving a letter through the post within the next few weeks. If you do not recieve a letter and you fit the criteria please call the surgery and check with the receptionist.

Practice Email

We now have a way that you can communicate with the practice via email.

This can be used for general queries or requests you may have. We still reccommend that you use patient access to order your prescriptions/ book appointments etc as it is a more secure way of doing so. Please note this is email is not manned 24/7 and will be answered throughout a working day. This is also not appropriate for clinical questions and should only be used for admin purposes.

Practice Demand

Due to the number of phonecalls the practice is recieving, we are sometimes limited to the service we can provide to our patients. To try and stop this from happening further, it is reccommended that you try to order your prescriptions/book appointments online or in person at the Surgery. This could potentially free up the amount of calls we are receiving which will be able to help us with the service that we provide to you as patients. Thankyou for your understanding.

Viewing medical records online

You can now view parts of your medical record online through Patient Access. For further information speak to the receptionists at the surgery or download our leaflet in the 'documents' tab.  



Saturday morning surgeries

To try and help the availability of appointments and to improve patient services, we are currently trialling a GP surgery on Saturday mornings. This is held at Churchfield Surgery and is for members of all the surrounding surgeries.

There will also be a Practice Nurse and a Healthcare assistant.  To book into these clinics please call the surgery to book. The surgeries start at 8.30am and finish at 12.00pm. There will be a different clinican each week.


Please note we will not be able to dispense any prescriptions in these surgeries as the scheme is not run by this surgery.


THIS IS NOT A WALK IN SURGERY. You will only be seen if you have booked an appointment previous.
